Student Forms Portal (Online Forms)
The Student Forms Portal is a central location that allows the student (& parent, if necessary) to easily resolve FAFSA Verification, SAP Appeals, and other requirements as well as submitting the requests below.
How to Submit a Request:
- First-Time Users: Create an account on our Student Forms Portal by using your SUNY Delhi student email. Be sure to enter your name and any other requested information exactly as it appears on your FAFSA.
- Once an account has been created, students will log in using Single Sign-On (SSO) with their Delhi credentials at the Student Forms Portal.
- Once logged in, click on 'Manage Request' and select the form that best relates to your situation.
- If a parent signature is required, the student will be prompted to invite the parent to create an account. This will allow a parent to review the form and e-sign.

- Special Circumstance: Financial Situations
Students and/or their parent or spouse who have experienced a change in their financial situation may request a re-evaluation of the student's federal financial aid eligibility. If the student already has a 0 through -1,500 SAI (from the FAFSA), do not complete this form as the student was already considered for the maximum Pell and Subsidized Loan. - Special Circumstance: Cost of Attendance (COA) Increase
Students who need to request an increase to their annual Cost of Attendance (COA) may submit this appeal. Appeal is coming soon for 2025-26.

Unusual Circumstance: Request for FAFSA Independence
For dependent students who are requesting to be independent for federal financial
aid due to an unusal circumstance.
Additional Unsubsidized Loan: Parent PLUS Loan Override Request
If the parent of a Dependent Student is unable to borrow a Parent PLUS Loan, the student
may request to borrow an additional unsubsidized loan. This form must be completed
by both the student and the parent. Supporting documentation will be required.
Note: Alternatively, a parent may apply for Parent PLUS Loan and if credit denied, the student would have the option to borrow an additional unsubsidized loan instead of submitting a request above.
